AFFIDAVIT OF UNDERTAKING
I, SALVACION D. CHUA, Filipino, of legal age, with residence address at
Tara St., Barangay Pating, Masbate City, after having been duly sworn to in
accordance with law, do hereby depose and state:
1. That I am the owner of RANCHER’S TOURIST INN at Tara St.,
Barangay Pating, Masbate City;

2. That the Fire Safety Inspectors of the Bureau of Fire Protection-

Masbate City Central Fire Station has conducted fire safety inspection
on my above-stated building and they found out that there were fire
safety requirements needed to meet under RA 9514 otherwise known
as the Fire Code of the Philippines;

3. That I hereby undertake to comply with the following requirements

within eight (8) months:
a. Provide/Install Automatic Fire Suppression System/Sprinkler
System
b. Provide Fire Detection & Alarm System (FDAS)
c. Install Appropriate Type of Stair Exit
d. Install Illuminated Directional Exit Signage
e. Submit Certificate of Electrical Testing

4. That in the meantime, I am taking full responsibility if ever fire

occurrence and other related disaster shall damage or destroy my
property.
IN WITNESS WHEREOF, I am executing this Affidavit to attest the truth
of the foregoing facts this 27 th day of April 2021 at Masbate City, Masbate,
Philippines.
