Greetings!

I would like to ask your permission to conduct a Home Visitation to one of my pupils who have
been absent for almost 3 weeks, Frix Morgia of Grade 4 Aquino. According to one of his
classmates, Clark Kent Ravales, a neighbor, Frix’ grandmother was hospitalized and the family
had to take care of her in the hospital. When I asked Clark again last week, he said he no longer
see Frix and his family at their home and around the neighborhood. With this, I would like to
confirm and see for myself the situation of the family. Should this visit be approved, I will be
going there at _______________ with Clark Kent at 12:00 NN right after our class.
