AFFIDAVIT
of Parental Consent

I, RENE PAOLO M. ALMONIA guardian of ALLAN M. ALMONIA of legal age,

resident of Palinpinon, Valencia Negros Oriental and student of Negros Oriental State
University (College of Criminal Justice Education)after having been duly sworn to in
accordance with law, depose and say:

1. That I am giving full consent and authority to my brother ALLAN M. ALMONIA

to undergo On-the-Job Training for 270 hours which will be conducted to refutable
and partner law enforcement agencies and other components of the Philippine
Criminal Justice System this first semester, school year 2022-2023 in Dumaguete City
and the province of Negros Oriental.

2. That I have considered the benefits that my brother will derive from her internship
program provided that due care and precaution will be observed to ensure the comfort
and safety of my brother and that CCJE NORSU Employees and personnel may not
be held responsible for any untoward incident that may happen beyond their control.

3. That I am executing this Affidavit to attest to the truthfulness of the foregoing

statements made in this affidavit and for all other legal intents and purposes it may
serve.
