Indiana Child Mental Health Parent Survey

Characterizing Parent Perceptions of Indiana-based Pediatric Mental Health Services for Autism
Spectrum Disorder

Investigators at Riley Hospital for Children and Indiana University School of Medicine are conducting a
study to understand the needs parents have when helping their child with Autism Spectrum Disorder
access Indiana-based mental health services.

You may be eligible to participate in this research if:

• you are the primary parent/legal guardian of a child who is 5-17 years old and diagnosed with
Autism Spectrum Disorder,
• Your child received mental health therapy in Indiana at a Community Mental Health Center, and
• You are 18+ years old.

Participation in the study involves completing questionnaires asking about your child’s symptoms,
mental health services your child has received, and other interventions you have tried for your child’s
symptoms. The questions should take you about 20 minutes to complete.

Compensation: You will have the option to provide your email address to receive a $35 electronic gift
card after completing the survey.
