AFFIDAVIT OF LOSS

That I, LORIEMEL ABUAN SOMERA, of legal age, Filipino citizen, with residence
address at Block 27, Lot 40, Lauan Road, Barangay Saimsim, Calamba City, Laguna,
Philippines, after having duly sworn to in accordance with the law do hereby depose and
state that:

That I am a holder and in possession of Certificate of Employment of the following

companies:
 Soledad Marzo Galvez
 EMD Technologies Philippines I
 Peerless Integrated Services

That the documents together with other personal belongings were lost due to
unavoidable circumstances when it was unfortunately washed away by flooding;

Diligent effort has been made but futile; therefore considered it lost for all intents
and purposes;

I am executing this affidavit to attest to the truth of foregoing facts, for information
and guidance of proper authorities concern and for whatever legal purpose this affidavit
may serve.
