I, ISRAEL DAG-UMAN ENRIQUEZ, Of legal age, Filipino, married, and a resident of

Purok-7, Poblacion, Barobo, Surigao del Sur, Philippines, after having been sworn in
accordance with law, hereby depose and say:

1. That I am the same person who executed this affidavit;

2. That I have a plan to avail an agricultural loan with the LANDBANK OF THE
PHILIPPINES ( LBP );
3. That I hereby attest and affirm under oath that I have no outstanding
loans/obligations from any banking institution for the same project upon
execution of this document;
4. That I am fully aware of the consequences of committing the act of perjury if I will
state any falsehood in this affidavit;
5. That I executed this Affidavit to attest the truth of the foregoing facts and for any
legal purposes this may serve best.
