1. Title must bear the subject, grade/year level, and the quarter.

2. TOS must be one page only, portrait, font Arial, font size 9, the margin has been set already.

3. 1st column, write all the learning competencies assigned to you.

4. 2nd column, you are only allowed to use four common item formats. These are Multiple Choice Test,
Alternative Test (True or False), Matching Type Test, just choose one to either Identification Test or
Completion Test. This time, go back to the task I gave about “Guiding principles in Constructing Test
Items Using Different Item Formats”. This can surely help you. I just hope also that you have at hand on
this because this is your module 3. I just tasked you to have this in advance as reading materials.

5. 3rd column, go back to the message I sent particularly # 1 instruction. You can find again there, on
how to fill up this column.

6. 4th column, follow the formula.

No. of items = % of items x total number of items

7. 5th column, follow the formula.

% of item = specific number of days divided by total number of days times 100

8. In the column “Item Placement under each Cognitive Domain”, reflect the complete item numbers.
For example, if there are 4 items under Analysis level with item number 20-24 as reflected in the TOS, it
must be written as 20, 21, 22, 23 under Analysis level, not 20-24. This must be observed to all entries.

9. Follow the format on “Prepared by” part and do not forget your (e-)signature .
