We would like to ask permission to allow to conduct a survey among the “Non-Catholic
Students” in your class. This is in the view of our research, entitled “Impact to the Non-Catholic
Students on a Catholic school and their Anxiety Level: Senior High School”.
The survey would last only 5-10 minutes and would be arranged at a time convenient to the
students’ schedule (e.g during break time). Participation in the survey is entirely voluntary and
there are no known or anticipated risks to participation in this study. All information provided
will be kept in utmost confidentially and would be used only for academic purposes. The names
of the respondents will not appear in any chapter of the research study.
