2.6 NBI Clearance;
2.7 Medical Certificates;
2.8 Certificates of specialized training, if any
2.9 Certified copy of Voter's ID and /or any proof of residency as
deemed acceptable by the School Screening Committee
3. By executing this OMNIBUS CERTIFICATION OF AUTHENTICITY AND
VERACITY OF DOCUMENTS, I hereby authorize the Department of
Education, Division of El Salvador City, to verify the authenticity of the
abovementioned documents.
