This letter is to formally request Medical Claim for the medical expenses for my Insurance id -
0717967 I was visiting KVR Hospital , Hyderguda on 04-10-2022 where my surgery was happened.

The KVR Hospital Management wasn’t arranging the certificate that you mentioned.
The surgery was happened on last month so they are conveying that the certificate will issue before
surgery , so I haven’t got any information about the certificate from your end . so kindly claim the
insurance or suggest us any alternatives to claim.

I request you to kindly approve health insurance policy for which I shall be grateful to you.
