Dear Sir;

This is to inform your good office that I Adolfo M. Ortiz would like to apply for National
TVET Trainer Certificate (NTTC).

I have finished the required course for Trainers Methodology I and I have undergone
National Assessment and received the NC II level Certificate for Shielded Metal Arch Welding.

The said document will be used to comply with requirements of TESDA that all trainers
should have the NTTC Certificate. It shall serve as a proof that I am qualified to teach and assess
on the said qualification.
