ETHIONAMIDE is Correlated with Hypothyroidism

Ø Thionamide and PAS (para-aminosalicylate sodium) based MDR-TB regimens have been
associated with the development of primary hypothyroidism

Ø Mostly the reports of the hypothyroid case in MDR-TB treatment are originated from resource
poor countries, such as India and some African nations, where TB and human immunodeficiency
virus (HIV) are endemic and where other risk factors such as antiretroviral drug use, nutritional and
iodine deficiencies may also contribute to thyroid status.
Ø the incident reports are vary between 3.9% - 78%

Ø Ethionamide can lead to hypothyroidism by preventing the formation of thyroid hormone by


inhibiting the organification of iodine. Similar to methimazole, a drug that is used for treating
hyperthyroid.

Ø Ethionamide has similar structure with propylthiouracil (PTU) and methimazole which are
hyperthyroid medication, in which they are all come from thionamide group. This where the theory
of correlation between the use of ethionamide and the reversible hypothyroid in MDR-TB comes
from.
