BATONBATON, of legal age, Filipino, and residents of Talibon, Bohol, on oath hereby
state that:

1. We are the good friends of ALECIA TORTUGA AUXTERO/ALICIA T.

AUXTERO and we have known her for a long time;

2. That we are not related by consanguinity or affinity to said ALECIA TORTUGA

AUXTERO/ALICIA T. AUXTERO;

3. That the real and legal name of said ALECIA TORTUGA AUXTERO/ALICIA
T. AUXTERO is ALECIA TORTUGA AUXTERO, based on her National ID
which is a government issued ID;

4. That ALECIA TORTUGA AUXTERO and ALICIA T. AUXTERO are one

and the same person who is our friend, ALECIA TORTUGA AUXTERO;

5. The name “ALICIA T. AUXTERO” that is on her “Certificate of Full Payment”

issued by CAP INSURANCE, is an error resulting from excusable inadvertence;

6. I execute the forgoing to attest to the truthfulness and veracity thereof and for
whatever legal purpose this may serve;

7. I execute the same freely and voluntarily and fully aware of any liability should
the forgoing be proven false or perjurious.
