Proclamation requiring the attendance of a Witness

Sections 82, 87, and 90 of Code of Criminal Procedure, 1973

WHEREAS complaint has been filed before me stating _________ (personal details, name,
address) has perpetrated (or is suspected to have perpetrated) an unlawful act of _________
(concisely mention the act) and a warrant has been served to urge the attendance of
_________ (personal details, name, address of the witness) before the Hon’ble court to be
investigated relating to the aforesaid matter of the complaint ; and whereas it has been given
back to the aforesaid warrant that the said _________ (name of witness) cannot be served,
and it has been revealed to my contentment that he has escaped from custody (or is obscuring
himself to avoid the service of the aforesaid warrant);

Proclamation is hereby made that the said __________ (name of witness) is called for
