The Alameda County Social Services Agency has submitted its Final Report to the Court in the matter of
your adoption petition. Copies of our report and all pertinent documents for your review are enclosed.

Our report is a conditional approval, as there are outstanding parental rights, which need to be addressed
before requesting a finalization hearing. Please take the necessary steps to address those rights in Court.

Once the remaining parental rights have been terminated in Court, you may call (510) 690-2745 or email
FamilyLawCivilAdoptions@alameda.courts.ca.gov to schedule court hearing date, if the Court did not
already finalize the adoption.

Immediately following the Finalization Hearing, please furnish us with a filed copy of the Adoption Order
(ADOPT-215), copy of the petition to terminate parental rights, and a copy of the approved order
terminating parental rights.
