MATERIALS/EQUIPMENT: (Gloves, blanket or

cover kit/shroud, Washcloth, ties and tags, clean
gown, Extra under pads/diaper, Bath kit, paper tape,
gauze pads/strips, Scissors)
1. Gathers all materials needed.
2. Checks time of pronounced death from chart.
3. Closes door/window/curtains.
4. Allows time for the family to grieve/ pray
momentarily.
5. Informs the supervisor and census department.
6. Explains the procedure to the bereaved family.
7. Put on clean gloves.
8. Removes jewelries and all valuables of the
patient.
9. Puts to a bag, labeled properly and returned to
the immediate family.
10. Positions the patient into supine.
11. Straightens the limbs gently.
12. Closes the eyes gently using the hands.
13. Returns dentures properly, if patient uses one.
14. Places a rolled-up washcloth under the chin.
15. Places a fresh pillow under the patient’s head or
raise the head of the bed about 30 degrees.
16. Places absorbent under pads/diapers under the
buttocks.
17. Removes catheters and tubes inserted into the
patient. (IV, IFC, NGT)
18. Covers the insertion sites with cotton and
plaster.
19. Cleanses the face beginning from the eyes.
20. Cleanses the body thoroughly.
21. Puts a tag (Indicating name of patient, age, date
and time of death) on the left wrist.
22. Puts a tag on the right big toe.
23. Wraps the patient snugly in the blanket/shroud.
24. Places a tag on the ankle area.
25. Facilitates transfer of patient to morgue.
26. Does after care of materials used.
27. Removes gloves.
28. Washes hands.
29. Documents. (Time of cessation of Vital Signs,
Persons notified and time of notification, List body
removed from unit, Destination and by whom
removed)
30.Communicated therapeutically to the patient and
family member/s during the procedure
