The situation is about the nurse work in a multidisciplinary ambulatory care clinic and
diagnostic centre. Nurse giving the bad news to the patient that has been diagnose with breast
cancer. The nurse’s name is Betty. The patient’s name is Mary Smith, she came alone because
her husband is out of town for a bit. The scenario for the result is very overwhelming.

The nurse’s word was correct. Nurse was use SPIKES protocol.

SPIKES Protocol
S (Setting up the interview)
1) Greetings
Nurse: Hi, good morning. Come in, please
Patient: Hi, good morning, thank you
2) Introduction
Nurse: My name is Betty; I am the nurse working in the clinic today. What’s your
name?
Patient: Yes, my name is Mary Smith, like a common
3) Seating
Nurse: Smith is pretty common, that’s good idea. Please have a seat.
Patient: Yes, thank you
4) Confirming ID
Nurse: What is your name and date of birth?
Patient: My name is Mary Smith, my date of birth is June 10,1955
Nurse: Perfect, the reason I asked is to make sure that if correct patient
Patient: Okay

P (Perception of the patient)


1) Purpose of visit
Nurse: Can you tell me why you are here today?
Patient: Oh well, I had breast biopsy a week ago. I am here to get the result of the biopsy
test. You know right?
2) Knowledge and expectation about the news (from family doctor, others)
Nurse: Yes, I know, I just want to see what your expectation. Did you speak with your
family doctor before you coming today? Did the doctor tell you about this?
Patient: I did not speak because he is on vacation. I did not have a chance to speak. Do
you have some information?
Nurse: Yes, I do, but I just make sure that your family doctor did not speak to you, so I
will be giving you the information about the pathology result. Is there anyone with you
here today?
Patient: No, I come by myself, my husband is out of town for a bit.

I (Invitation by the patient)


1) Show the result
Nurse: Let me get my information here. This is a pathology report from your biopsy.
Patient: Sure, what is the result?
2) Invite to choose detail or diagnosis
Nurse: Let’s just move little closer together. Would you like me to start with the details
or the summary?
Patient: Just give me the summary

K (Knowledge/Information to the patient)


1) Show the information
Nurse: This is a pathology report from your biopsy that you had.
2) Apologize
Nurse: Unfortunately, this what it seems the plan today, to inform you and then to help
you along the path that diagnostic has given you.
3) Breaking bad news
Nurse: Lets go down to the bottom part here, it says you do have breast carcinoma
cancer. Were you expecting that? You did not plan for this in your mind today?
Patient: No, I know there was a possibility, but I was not hoping in that case.

E (Emotions of the patient)


1) Emphatize
Nurse: I know this is not the news that you wanted today. I know this is difficult.
Patient: Yes, why is this happening to me?
2) Offer to call someone
Nurse: Do you have anyone at home? Or near you? I know your husband away, that is
very helpful to you to give you some support, so you’re not alone
Patient: No, thank you. Just give me sometime to calm myself down. I had to think
about it.
3) Calm down
Nurse: That’s okay, you can think go home, decide later and tell the doctor.
Patient: Thank you
Nurse: If you find it is too overwhelming and you don’t want to deal wit it, it si your
decision. But it is here that you need about information about the surgery.
Patient: That is very good

S (Strategy and summary,plan of action).


1) Share optimism
Nurse: Invasive ductal carcinoma that is a cancer that starts in the milk duct and grows
into the normal breast tissues, it is the most common types of breast cancer so it is there
are many women that would have this as well. Commonly, there is good treatment for
that.
Patient: Yeah, I hope for that
2) Share available options
Nurse: The doctor will come to see you, examine on you, and explain some of the
options that are available for you. Explain some surgical option to you.
Patient: That’s very good
Nurse: I will give you some information between lumpectomy and mastectomy. This is
the clinical practice guidelines for treatment of breast cancer from the Canadian
Medical Association. If you find it is to be overwhelming and you don’t want to deal
with it, it is your decision. But here that you need information it is available.
Patient: Such a good source
Nurse: Yes, it is helping you to choose the most appropriate operation. Ask the
questions about mastectomy and what is lumpectomy, the advantages and
disadvantages of it.
3) Prepare to meet the doctor
Nurse: When you are ready, I will take you to see the doctor.
