A Case of Idiopathic Thrombocytopenic Purpura After Booster Dose of

BNT162b2 (Pfizer- BioNTech) COVID-19 Vaccine

SUMMARY:

The Covid-19 pandemic's incidence and fatality are currently thought to be best

prevented by vaccination. Almost all vaccines are considered safe, with the exception of

minor and rare side effects. Among the most common reported complications from the

COVID-19 vaccines are vaccine-induced thrombotic thrombocytopenia (VITT)/

thrombosis with thrombocytopenia syndrome/vaccine-induced pro-thrombotic immune

thrombocytopenia syndrome. Moreover, we found a patient who acquired idiopathic

thrombocytopenic purpura (ITP) after getting a booster dose of the Pfizer Covid-19

vaccine. The patient had a history of scleroderma and rheumatoid arthritis and was

receiving methotrexate and hydroxychloroquine therapy. After receiving the booster dose

of the vaccine, she presented to the emergency hospital four days later with

thrombocytopenia and purpura.


A 75-year-old female who received an uncomplicated first and second

immunization doses but developed VITT after getting the booster dose. At the baseline,

she was never diagnosed with thrombocytopenia or any other platelet disorder.

Furthermore, the patient was found to have a low platelet count of 75,000/mm3, which

dropped to 18,000/mm3 after five days, at which point the patient was sent to the

emergency room for additional evaluation. In addition to receiving a unit of platelet

transfusion, the patient was started on a daily dose of dexamethasone 40 mg by mouth.

Her platelet counts improved gradually.

REACTION:

The goal of this study is to raise awareness of potential problems, which will lead to

early discovery and treatment, rather than to lower the safety criteria of the Pfizer-

BioNTech COVID-19 vaccine. In the present era, the most effective weapon against the

fatal COVID19 pandemic is m-RNA and various vaccinations. The signs and symptoms

of vaccine-induced thrombocytopenia should be acknowledged by clinicians. As in this

case, it is clear that patients who experienced no side effects after receiving the first and

second doses could get thrombocytopenia following the third dose. Despite tolerating the

regular dosages or past booster doses without complications, any of those boosters has

the ability to cause Idiopathic Thrombocytopenic Purpura. A delay in diagnosis and

treatment could result in serious bleeding complications.

As a student Nurse, I am encouraging everyone to get vaccinated. Vaccines and

Boosters are a crucial component of self-defense from acquiring COVID-19 and dying as

a result. They are advised for the majority of the population.


As a nursing student, I should not only look at one aspect of my patient and should

treat my patient as a system of all connected factors including physically, spiritually,

emotionally and psychologically. A lot of nursing theories also implies that. A failure to

do so makes you a nurse who tends to patients as an object that should be repair and

not a person who needs healing. This case thought me not just medical conditions of

people who are in lean sports but also of how biased people still look for the enforced

beauty standards in our society always going for that slim, petite and curved body build.

Also, as I am I do not just to read that and putting myself into a nursing student to speak

out, me as a person especially born in this generation would like to encourage everyone

that we should accept differences regarding race, gender preferences in life, principles,

outlooks and body type. Lastly, with the words I heard I quote, "as society we need to

release ourselves from the authority that we tend to exercise over other people's lives".
