Thank you for accepting Elenor, an 8-year-old primary school student, who is being transferred to
paediatrics ward today to access delayed problems and speech therapy. She was admitted to our
hospital with a diagnosis of potential hypoxia.

Elenor had a past history of fractured tibia at the age of 4. Also, she has been suffering from mild
eczema.

On 24/09/19, Elenor presented with a complaint of falling into the freshwater lake which was found
after 15-20 mins. On assessment, her body temperature was 27.7C along with pulseless, cyanotic and
apneic. In addition, CPR was performed and a paediatric ventilator and the endotracheal tube were
inserted to achieve spontaneous circulation.

During hospitalization, Elenor’s condition was stable and she was transferred to a general ward. She had
not responded to command for basic things.

On today’s review, Elenor has some difficulties in concentration and possible dysarthria for which
speech therapy was commenced. Also, paracetamol was prescribed for headaches. Therefore, she will
be transferred to your ward to continue speech therapy, check for delayed problems, organize
neurological exams, and monitor cough, headaches and dysarthria. Her parents have been discussed the
possible sequela due to hypoxia.
