Re: Mr. Yanlin Ma, DOB: 12/03/1986

I am writing to inform you regarding Mr. Ma, a 33-year-old international Ph.D. student, who is being
discharged after successfully undergoing treatment of open chest surgery for repair of dissection and
aortic root replacement with a mechanical valve.

Mr. Ma is a heavy smoker and his BMI is 30. He has been suffering from hypertension since 2016.

On 31/03/19, Mr. Ma was admitted to our hospital with severe chest and back pain. His CT scan
revealed severely dilated ascending aorta with type A dissection and her echocardiogram indicated AI
and aortic valve incompetence. Therefore, open chest surgery was performed.

On today’s review, Mr. Ma’s condition had recovered well with no post-operative complications. His
discharge medications are amoxicillin and clavulanic acid 875mg/125mg, warfarin 3mg, amlodipine
10mg, clonidine 100mg, isosorbide mononitrate 60mg, metroprolol 100mg, ramipril 10mg, temazepam
10mg, oxycodone 5mg, and Panadol 500mg. In addition, He was advised to do routine wound care,
maintain his INR result at 2.5-3.5, monitor his blood pressure, continue warfarin therapy, reduce his
weight loss and quit smoking. Also, he was referred to cardiac rehabilitation to review his exercise
tolerance.

In view of the above, I would be grateful if you could follow up on Mr. Mar’s condition. Please contact
