Re: Mr. David Taylor, 38 years of age

I am writing to refer Mr. David Taylor, a 38 -year-old landscape gardener, who is


presenting signs and symptoms of a damaged medial cartilage in the left knee joint to
prevent future osteoporosis. Your further management and assessment would be
appreciated.

Please note that he is an active person who enjoys sports.

On 12/08/10, he presented to me with a complaint of joint pain and swelling in the left
joint along with difficulties in strengthening the leg which started 6 months prior. Initially,
he twisted his left knee joint in a game of tennis. On examination, there were a slightly
swollen left knee joint, and a tender spot on media aspect of the joint without effusion.
Therefore, X-ray was ordered and Voltarin 50mg was prescribed.

On subsequent visit, he reported that there were intermittent attacks of pain and
swelling of the left knee joint. Additionally, there was impaired range of power in passive
and active movement.

On today’s visit, his symptoms was still persistent and he had an anxiety to maintain full
time work. MRI result revealed a damaged medial cartilage.

Due to the recent manifestation, I believe that Mr. Taylor needs specialist interventions
to remove damaged cartilage in order to prevent future osteoporosis.
