Re: Mr. Julian McDonald, DOB: 12/01/50

Thank you for accepting Mr. McDonald, a 68-year-old man, who has recently undergone post total left
knee joint replacement for rehabilitation and assessment for suitability to return to his home.

Mr. McDonald lives alone and he is a heavy smoker. He has been suffering from osteoarthritis, gout and
hypocholesteraemia during the past 10 years. Also, his BMI is 35. Please note that he has an allergy to
penicillin.

On 20/07/18, Mr. McDonald underwent a joint replacement surgery with a specialist Mr. B Mossley.
After an operation, he reported that he had difficulty in urination for which amitriptyline was prescribed.
Also, his CSU showed Staphylococcus saphroluticus for which Keflex was given.

Regarding his discharge medications, there are Zyloric 300mg, Lipitor 20mg, Karvina 300mg, Mogadon
5mg, paracetamol 1mg, ibuprofen 400mg, Nicabaate 21mg, Targin 20/10 and Oxycodone 5-10mg.

On today’s review, Mr. McDonald is ready to be discharged into rehabilitation care including
physiotherapy and occupational therapy. Therefore, the possibility of drug, alcohol and sleep study will
be arranged into your care. Please note that his suture needs to remove in the next 10 days and his next
appointment with Dr. Mossley on 07/09/18.
