Re: Ms. Ling Wu


DOB: 01/03/2001

I am writing to inform you regarding Ms. Wu, an 18-year-old female, who is being discharged after
successfully undergoing treatment of a knee amputation due to a cycling accident. Your follow-up would
be highly appreciated.

On 08/04/19, Ms. Wu was transferred to the Emergency Department. Her GCS was 15 and her blood
pressure was 178/90 mmHg and her pulse rate was 140bpm. Her examination revealed an open tibial-
fibular fracture with multiple contusions. Therefore, the above-knee amputation was performed on the
same day.

During hospitalization, Ms. Wu reported phantom limb pain and insomnia for which Endone 5mg,
amitriptyline 10mg, and Neutotin 100mg were prescribed. Also, she had social withdrawal anxiety due
to a fear of public workers knowing about her condition. On assessment, her amputation’s incision was
healed along with good mobility which assisted with a rolling walker. Therefore, Paracetamol-Osteo 665
mg and tramadol 100mg were prescribed.

Regarding Ms. Wu’s discharge plan, she has been trained to wrap stump with ACE bandage and
arranged a CBT program. Therefore, I recommend you to monitor for the signs of PTSD and remind her
to review at Trauma Clinic on 17/05/19.
