Declaration

1. I/ we do hereby declare that above particulars of information and facts stated are true, correct and
complete to the best of my knowledge and belief.

2. I / we declare to abide by Standard Operating Procedures (SOP) of Ministry of Home Affairs, Order
No. 40-3/2020-DM-I (A) dated 05.05.2020 & OM dated 22.05.2020 regarding repatriation and travel
abroad of Indian seafarers.

3. I / we declare that the seafarers have maintained travel history and daily temp. records as per DGS
Order 12 of 2020 SOP for controlled crew change and are medically fit for sea service, healthy with no
symptoms of COVID-19.

4. I / we declare that the chartering agency would provide manifest of the Indian seafarers to be
transported to a destination outside India prior departure to DG Shipping;

5. I / we declare that the cost of accommodations, transport, access to medical treatment for existing
morbidities or for any COVID-19 infected cases and its cost need to be borne by ship owner / RPSL
agency as per applicable Collective Bargaining Agreement (CBA).

6. I / we declare that the onus for the seafarer clearance through immigration either in transit or at the
destination country lies entirely with us. If entry or permissions is denied either in transit or at the final
destination, then the entire onus is on us to repatriate the concerned seafarer to India.

7. I / we also declare that, after reaching the desired joining country, seafarer would abide by the
quarantine / isolation and health protocols as desired by the on-signing country / on-signing port / flag
state as applicable.

8. I / we declare and undertake that seafarers on arrival to destination airport (India) and subject to
quarantine requirements, the entire responsibility of cost for seafarers’ quarantine stay, would be borne
by us.

9. I / we also declare that the seafarers shall wear necessary PPE, abide by social distancing norms at all
times and all applicable travel abroad guidelines as per the Ministry of Health & Family Welfare
(MoHFW), Govt. of India and relevant guidelines.
