As part of the study entitled, “THE IMPLEMENTATION OF DISTANCE LEARNING IN
SENIOR HIGH SCHOOL MATHEMATICS OF TABACO CITY DIVISION”, this questionnaire will
gather data on the implementation of distance learning along: (a) learning modalities, (b) learning
resources, (b) monitoring, and (b) learning assessment. Your experiences and insights as Senior High
School student will be solicited to recommend suggestions in the improvement of the implementation.
Please feel free to answer the questionnaire and provide the needed data. Rest assured that the
information will be treated with utmost confidentiality, and will only be used for this purpose.

Indicators
LEARNING MODALITIES.
This section contains indicators that would describe the delivery of the learning modalities implemented
by the school.
Asked to answer survey form before the modular distance learning
implementation.
The school has provided me with enough modules and other printed
materials for my MDL.
LEARNING RESOURCES.
Aside from the usual textbooks, Self-learning modules (SLMs), Learning Activity Sheets (LAS), and other
materials in digital and print copies can help reach learning goals appropriate to their needs.
Learning modules, activities, and other activities given by teachers
are designed to achieve the learning competencies.
Learning Assessment
This section contains the assessment and grading practices executed by teachers and schools which are
in line with DepEd Order No. 31 s. 2020.
Guidelines and other important details on assessment and
examination are communicated to me by teachers.
There are quizzes, the long test provided before the summative
assessment.
The assessments given evaluate our learning aligned with the desired
learning competencies.
