SURVEY QUESTIONNAIRE FOR SENIOR HIGH SCHOOL STUDENTS

(This is for schools catering Modular Distance Learning (MDL))

Dear Respondents,
As part of the study entitled, “THE IMPLEMENTATION OF DISTANCE LEARNING IN
SENIOR HIGH SCHOOL MATHEMATICS OF TABACO CITY DIVISION”, this questionnaire will
gather data on the implementation of distance learning along: (a) learning modalities, (b) learning
resources, (b) monitoring, and (b) learning assessment. Your experiences and insights as Senior High
School student will be solicited to recommend suggestions in the improvement of the implementation.
Please feel free to answer the questionnaire and provide the needed data. Rest assured that the
information will be treated with utmost confidentiality, and will only be used for this purpose.

II. INDICATORS ON DISTANCE LEARNING (DE) IMPLEMENTATION

As a Senior High School student, how will you assess your school in terms of learning
modalities, learning resources, monitoring, and learning assessment.
Please respond to the following statements using the 5-point Likert scale to indicate to which
level you agree. Tick the box which corresponds most closely to your desired response.

Indicators
LEARNING MODALITIES.
This section contains indicators that would describe the delivery of the learning modalities implemented
by the school.
 Asked to answer survey form before the modular distance learning
implementation.

 There are programs, projects, and activities implemented by the


school that assist my modular distance learning (MDL).

 There is support from other stakeholders that helped me cope with


modular distance learning.

 The school has provided me with enough modules and other printed
materials for my MDL.

 Answers the activities in modules at my own pace.

 Distribution and retrieval of modules are scheduled weekly.

 Weekly Home Learning Plan or instructions on weekly learning


activities is provided to me.

 Late submission of activities is considered by teachers.

LEARNING RESOURCES.
Aside from the usual textbooks, Self-learning modules (SLMs), Learning Activity Sheets (LAS), and other
materials in digital and print copies can help reach learning goals appropriate to their needs.
 Learning modules, activities, and other activities given by teachers
are designed to achieve the learning competencies.

 There are no errors in the modules, activities, and worksheets.

 The content of the modules and activities are relatable.

 The learning modules, activities, and other worksheets help me


develop my critical thinking skills and creativity.

 The modules and activities were learned independently.

 The learning resources are provided to each of the students.

 The learning resources are distributed regularly in the distribution


centers at school or at the partner barangay center.

MONITORING OF DISTANCE LEARNING IMPLEMENTATION.


This section has information on how the school adopts the new normal and how it adheres to the health
protocols imposed by Inter-Agency Task Force Guidelines.
 The school has a COVID-19 Task Force that monitors the school
implementation of modular distance learning.

 There is regular monitoring of module distribution and retrieval of


activities from parents/learners.

 Teachers send letters/messages regularly to check our progress in


activities.

 Reminders on deadlines are sent via messenger/letters or during the


schedule of distribution and retrieval of modules.

 Teachers conduct home visitation to check my performance in MDL.

 There is communication support via messenger, and other platforms


whenever I have difficulty in MDL.

Learning Assessment
This section contains the assessment and grading practices executed by teachers and schools which are
in line with DepEd Order No. 31 s. 2020.
 Guidelines and other important details on assessment and
examination are communicated to me by teachers.

 Teachers provide different tools and modalities of assessment to


evaluate our learning in modular distance learning.

 There are quizzes, the long test provided before the summative
assessment.

 The assessments given evaluate our learning aligned with the desired
learning competencies.

 There are at least 4 written works provided by teachers in every


subject.

 There are at least 4 performance tasks with rubrics provided by


teachers in every subject.

 Results are sent to me at least 5 days after the assessment was


administered.

 Checked test papers are returned by teachers to me.

 There are intervention activities provided to me.

 There are assessments other than a written or paper-and-pencil test.
