This is with regards to my vacation leave schedule. I would like to respectfully request change of
my vacation leave schedule, I was not able to use them as scheduled because of the training of
new Hospital Information System (HIMS). The schedule are as follow:

September 16- 20, 2019 October 30- November 3, 2019 (5 days)
