Subject: Incident Report

On September 3, 2022, Mr. Grass was caught drinking on the job and placed on temporary suspension.
According to Mr. Grass, Ms. Owl supplied the alcoholic beverage as an attempt to remove him from
their shared office space. As a response, Ms. Owl states that Mr. Grass has been exceedingly disruptive,
therefore inhibiting her ability to perform to the standard at which is required. However, Ms. Owl denies
supplying any alcoholic beverages. Mr. Grass has denied any allegations of being disruptive and further
denies knowledge of the company’s “no drinking” policy.

In response to this incident, an official investigation has begun to determine the supplier of the alcoholic
beverage, as well as to determine if both parties have been briefed on the “no drinking” policy upon
employment. Mr. Grass and Ms. Owl have been removed from their shared office space and have been
placed on a no contact order while conducting business at Owl & Grasshopper Design Inc. Following the
findings of the official investigation, further disciplinary action will be taken.
