AFFIDAVIT OF LOSS

That I, EMMANUEL S. GONZAGA, of legal age, American Citizen,

married to Ruby De Leon Gonzaga and a resident of 14671 ½ Sherman way, Van
Nuys, California 91405, USA, after being having been duly sworn on oath in
accordance with law hereby voluntarily depose and say:
1. That my parents are the registered owner of a parcel of land situated in the
312 Luis Asistio St. Sangandaan, Caloocan City Metro Manila. As evidence
by TCT No. T-18452, issued by the Register of Deeds of Caloocan,
containing an area of ninety-nine (99), SQUARE METERS, more or less:

2. That I am one of the legal heirs of my parents, Felipe Gonzaga and Virginia
Sarmenta who are both deceased as evidenced by their death certificates
(Refer to Annex “A” – Death Certificate);

3. That my sister, CLEOTILDE GONZAGA DELLA who was in possession

of the title either lost or misplaced the title and she executed an affidavit of
loss on August 18, 2005 (Refer to Annex “B” – Affidavit of Loss);

4. That despite diligent search and effort exerted to locate/recover the same had
proved futile;

5. That the affidavit of loss was not annotated at the back of the title and have
not been filed in the Register of Deeds of Caloocan;

6. That I am executing this affidavit to attest to the above facts, and to request
the Honorable REGISTER OF DEEDS of Caloocan to issue the new
owner’s duplicate of Title for the TCT. No. T-18452;

IN TRUTH WHEREOF, I have here unto set my hand this ____ day of
