hereby authorize Olive M. Historillo to get a copy of my birth certificate from
Local Civil Registry Office (Boac) on my behalf as I currently unable to do so
myself since I am currently residing in Quezon City.
I am requesting a copy of my birth certificate as it is required to process
documents for my benefits in SSS, PhilHealth, ang Pag-Ibig.
