FEE: $400 Excl. GST

I appear voluntarily and there is no restriction or obligation which would prevent me from appearing in The
Production or which would restrict the use of any film of me being included in The Production. I also Confirm
the accuracy of my testimonial and my statements to be true & correct for the TVC

I hereby consent to and authorise DK Advertising Pty Ltd to use, reproduce, copy, exhibit, broadcast, screen
and distribute (in full or in part) for any purpose whatsoever any photographs or videos or recordings or
interviews or opinions in any other format whatsoever of my image and movements and/or recordings and/or
comments of my voice made in connection with the Project (including publicity and advertisements) and/or
written extraction (in full or in part) of such recordings, or computer files in which I may be included.

I waive any right to inspect or approve the finished product including any written copy that may be created in
connection with the Project. I waive this right noting that DK Advertising may, at its sole and absolute
discretion, amend or abridge or add to any material whatsoever featuring me in connection with the Project.

I warrant that, to the best of my knowledge, care and control, any interviews or opinions or representations
given by me (either directly or indirectly) arising out, during or in connection with the Project will not be false,
misleading, deceptive, indecent, defamatory, libelous, unlawful or otherwise prejudicial to DK Advertising’s

interests. I agree to indemnify, and keep indemnified, DK Advertising against all “claims”, damage, loss, cost or
expense of any nature that DK Advertising suffers or incurs (directly or indirectly) in connection with any
breach of the terms and conditions set out in this Deed.

I also hereby absolutely and irrevocably release and forever discharge, and agree to save harmless DK
Advertising and all persons acting under their permission or authority including the Producer from any
“claims”, liability or injury that may occur while performing or appearing in such video, audio, photographic or
computer-based production or other recording which but for the execution of this Deed I may have had against
DK Advertising.

Use of the term “Claims” above includes all claims, actions, suits, causes of actions, debts, dues, costs, claims,
liabilities, demands, damages, losses, costs and expenses of any description, decisions, awards, judgment and
orders whether at law or in equity or arising under statute arising out, during or in connection with the Project.

I have closely and carefully read this release. I have been given the opportunity of seeking independent legal
advice before signing it. I agree that it is reasonable and clearly understand and agree to be bound by its
contents.

the deed of release on behalf of the Performer / Interviewee.
