Draft Annex Protocols for Desludging Septic Tanks (Sub-Contractors) Pre-destudging Operations © Clean Liquid Philippines Inc, (CLP!) will notify the service provider the scheduled households and all necessary details + CLP) shall assign personnel that will be in-charge to over: services. + Tho service provider must be strict on time to avoid delay on providing services to other households. ts goal must be to accomplish the route plan. ‘+ The truck and all equipment to be used must be inspected before arriving at the desludging sit. ‘+ The service provider and its team must wear appropriate uniform, including but not limited to, Personal Protective Equipment (PPE) ike facemask/breath mask, rubber gloves, uniform/apron, gumboots, safely helmet and goggles and torch. 6 the entire scheduled desiudging During Destudging Operations + The owner or any member of the housshold, as well as the Barangay Captain or its authorized representative must be present on tho scheduled day of desludging to assist the service provider to the location of the septic tank and have full access on it. ‘+ At the outset of the desludging process, the truck operator and helpers must put on the Personal Protective Equipment (PPE) lke protective clothing, goggles, helmet, gloves etc. and make themselves protected from possible anticipated hazard. It must be ensured that all the safety equipment's are functional and good for use before proceeding to a septic tank. ‘+ Alter the sludge sucking is complete, the septic tank must be checked for remaining sludge. + Identity any abnormal conditions, such as high concentration of non-biodegradable materials, oils and grease before taking to the treatment plant for final disposal, Post-desludging Operations ‘+ Customer Care Questionnaire provided by CLPi must be signed by the HH after the desludging service is rendered. ‘+ The Service Manifest Form (SMF) provided by CLPi must be filed up and signed by the household owner, service provider and Barangay-Captain authorized representative after the desludging service is rendered. + The service provider must ensure that no trash or waste arising from rendering its service will be left on the site. Check for spills, if any. Add disinfectants solutions to the affected areas. Clean up any spillage using proper absorbent materials. + The service provider shall submit through email duly signed e-copy of SMF and Customer Care Questionnaire to CLPi, as well as proper documentations/pictures of desludging services, on a daily basis. The original copy of SMF will then be submitted upon pick-up of check payments. Penalty — 500.00 pesos per every finding.
