authenticated by the Barangay Chairman or his/her authorized official;
☑ Photocopy of the Valid Identification Card (ID) of the accused or any other
document for his/her identification

II. Additional requirements if the case is filed in other court

☐ Certified photocopy of the Information;
☐ Certified photocopy of the Warrant of Arrest (if voluntary surrender); and
☐ Certification that there is no judge in the Court where the case is
pending (unless voluntary surrender).
