PETITIONER, through the undersigned counsel and unto this Honorable Court, most respectfully alleges

that:

1. Petitioner is of legal age, married, Filipino and a resident of Anyani Road, Barangay Bari,
Mangaldan, Pangasinan, where she may be reached with notices and other legal processes of
the Honorable Court;
2. Respondent is of legal age, married, Filipino, and may be served with legal summons, notices
and other legal processes at 456 Dapitan Street, City of Manila;
3. Herein parties met sometime in the year 2012 while both were working at a garments factory in
Tondo, Manila;
4. Respondent confessed love at first sight with the petitioner, and the latter was enamored by the
affection showed by the former, hence, they became sweethearts after four (4) months of
courtship;
5. They decided to live together at a small rented apartment at 654 Piy Margal St., City of Manila,
until they got married in Pangasinan on June 12, 2014 and lived there thereafter (Copy of the
Certificate of Marriage attached as Annex “A”);
6. On May 10, 2015, petitioner gave birth to their son, Maynard (Copy of Birth Certificate attached
as Annex “B”);
7. Respondent continued to work at the garments factory where their love story began;
8. It was a perfect match made in heaven, or so petitioner thought. Her nightmare started when
respondent remits his salary substantially less than what he used to give. What used to be
respondent’s once-a-month vacation in Pangasinan became once-in-two months, until it
became longer;
9. Petitioner confronted respondent about the sudden changes in the latter’s behavior but
reasoned out his enormous works. Respondent had not shown any vices during their marriage;
hence petitioner somehow believed him;
10. Not long thereafter, she was amused by her discovery. Respondent left an Official Receipt with
tracking number of cash remittance issued by a remittance service provider (Copy of Official
Receipt attached as Annex “C”);
11. Respondent stirred up the hornet’s nest by inadvertently leaving a paper trail that would pin
him;
12. Unknown to respondent, petitioner made moves silently to uncover the truth behind her
suspicion. Petitioner called the contact number tracking number of the said Official Receipt, and
was shocked to hear from the other side that she was married to the respondent since August
21, 2009;
