    According to our records we have not received payment in full for the above captioned policy or any applicable premium changes
    made to the policy. Please remit your payment as soon as possible to avoid policy cancellation.

    If we receive payment of premium within 10 days from the date of this letter, the referenced policy will remain in force without
    any interruption. However if we do not receive payment of premium, we will cancel referenced policy and send Insured and Agent
    a formal notice of cancellation.
