HEALTH HISTORY

CHIEF COMPLAINTS:

Labor pain

HISTORY OF PRESENT ILLNESS:

The patient experienced early signs of pregnancy on January 24, 2022, at 8am in the
morning, with the symptoms such as nausea and vomiting during the first trimester, she noticed
also a darkening of her armpits and neck, and she also experienced frequent urination and
backache. In her second trimester, she felt pain in her stomach and fatigue. In her third
trimester, she did not experience any illnesses.

PAST HEALTH HISTORY:

She was asked about her gravida and para classification, we found out that the patient
is gravida 2 and para 2. She gave birth to all her children earlier than her estimated due date
and all of them are living normally, she also has no miscarriages and no multiple pregnancies.
She was also asked about her history of past illnesses, and it was revealed that she has no past
illness during her post-cesarean section on December 26, 2017 for her first baby. When it
comes to her immunization, she has completed all necessary vaccines. She has no known
allergies and has undergone one past surgical procedure, Cesarean section due to slow labor
progress.
