CUSTOMER’S CONSENT FOR AADHAAR OFFLINE E-KYC VERIFICATION

1. I have approached Hero FinCorp Limited (“HFCL”) for the purpose of availing loan from HFCL and in order to
avail such loan from HFCL, I am desirous of undergoing Aadhaar offline e-KYC Verification for identification
purpose (“Purpose”).

2. I hereby give my consent to HFCL for carrying out my Aadhaar offline e-KYC Verification (“Offline
Verification”) for the said Purpose. I hereby voluntarily and at my own discretion submit to HFCL, the offline
e-KYC XML as issued and digitally signed by UIDAI and the share code (“Offline e-KYC Data”), for the said
Purpose.

3. I fully understand that Offline e-KYC Data will include my name, download reference number, address, photo,
gender, date of birth/year of birth, mobile number (in hashed form), email (in hashed form) and I hereby give
my consent to HFCL for using my Offline e-KYC Data for the said Purpose.

4. HFCL has informed me the following details in respect of the Offline Verification:

a) The nature of the information that may be shared upon Offline Verification;
b) The uses to which the information received during Offline Verification may be put by HFCL;
c) The alternatives to submission of information, if any.

5. I hereby confirm and declare that all the information voluntarily furnished by me is true, correct and complete. I
will not hold HFCL or any of its employees/directors/representatives/agents responsible in case of any incorrect
information provided by me.

6. I hereby confirm and declare that I have clearly understood the contents of this consent form including the
Purpose of collecting, using and disclosing the Offline e-KYC Data and hereby giving my consent voluntarily
thereto.
