Vulvar Cancer:

 Irradiation is required if you have more than one microscopically positive node or a grossly
positive node. Enlarged nodes should still be removed because radiation will not cure bulky
nodes
 Melanomas that are less than 1mm in depth do radical local excision with clear 1 cm margin. If
between 1mm and 4mm then do radical local excision with 2cm margin and lymph node
dissection. If >4mm then no survival advantage of lymph nodes as distant metastasis are usually
present
