Chief Executive Officer CPGCL, GENCO.II, TPS, Guddu Subject: TRIPPING AND FIRE INCIDENT REPORT OF ST-16, 747 MW CCPP GUDDU Date & Time of tripping: 10-07-2022 & 0115hrs Weather Conditions: Heavy Rain Machine/ Plant Condition Prior To the Tripping at 0044hrs: * The Machine ST-16 was running on 210MW Load latched through both HRSGs (HRSG-I of GT-14 and HRSG-II of GT-15). The loading position was GT-14= 230MW and GT-15= 230MW, + Heavy rain was in full swing since about 0030hrs and dripping of the rain water was seen in the vicinity of Turbine Hall * Pre-incident routine round of the site was reportedly carried out by M/s Munawar Ali SSE (A),Ghulam Ali Operator Mehmood Ahmed Attendant, during the rain, at about 0030hrs. + All the panels were accordingly covered with Polythenes as a Precautionary measures Indications Appeared on HMI at 0044hrs: * 3 EXC. System UEL Limit active * EXC. System manaul Enabled «#3 EXC. Limit Auto to Manaul Hence,another round of the area was carried out. and the Excitation system was controlled manually. The situation was reported to Electrical Engineer Brief Description of the Event: In the meantime, at about 0115hrs, heavy bursting sound was listened and fire was seen on the Generator Excitor end presumably owing to: short circuit in Excitor travelling towards star point and Pts. System X For Latched. Prompt Action Taki Upon observance of the fire beside heavy breath’ chocking smoke underneath the Generator of ST-16, the following immediate actions were carriedout a. At first instance of the Fire appearance, the Hydrogen (Hz) released manually from the Generator by opening the CO, valve and correspondingly the He vent valve b. In parallel, first of all informed to Fire Fighting Officer and his team and also informed to all concerned including CEO, CPGCL TPS Guddu. ©. All the Axillaries of the Machine Stopped & Breakers racked out as, a safety measures. 4, Also both the running Gas Turbines GT-14 & GT-15 were stopped with NPCC Co-ordination as a safety measures at 0126hrs keep in the particular view of bare flames near GT-15, e. With all possible efforts of entire plant team and the fire crew, the Fire Extinguished / Controlled at about 0300hrs. f. Then the Lube Oil System energized / put in to service in order to protect Bearings, Shaft ete from further damage of ST-16 but the lube oil circuit was immediately stopped due to oil leakage from its Generator bearings. 9. After normalization of the alied system and getting clearance from all sections, GT-14 & GT-15 Started & Synchronized with system on open cycle mode as per NPCC. Demand at 0828hrs & 0928hrs, respectively h. All the concerned sections were directed for assesment of the damages and prepare rectification / rehabilitation plan of the Machine on war and foot basis in order to ensure minimum possible outage Apparent Damages Reported on ST-16, 747MW Guddu during the aforesaid Preliminary Assessment: INSTRUMENT & CONTROL SIDE: TG Control Panel, PLC, |O Module Primary Side Switches and Gauges Monitoring Cables H2 dew point Analyzer, Gauges. MECHANICAL SIDI As the Jacking Oil Pump Control Panel and Turning: Gear Motor and Circuit damaged the Lube Oil and TG failed to operate, the Mechanical damages on Bearing and Shaft cannot be ruled out.
