Affidavit of Financial Support

having been sworn in accordance with law, hereby depose and state that:

4. I undertake to support his/her financially with all of the

expenses relative to his/her delivery.

5. As his/her father, I hereby accept and assume any and all

responsibility for his or her welfare, including all financial
expenses for educational, medical, and all necessary expenses
for the upbringing of the child until he or she attain the age of
majority or is able to fully take care of himself/herself.

6. I am executing this affidavit, to support the mother of my child

financially in all the expenses she may incur in bearing and
rearing the child.

7. I also undertake to not cause any emotional, physical and

mental stress or anguish to the mother of my child, Vanessa
Jane Arcilla.

8. I also guarantee, to respect and recognize all the rights of my

child with Vanessa Arcilla, as conferred upon by the law, rules
and regulations of the Republic of the Philippines.

9. I guarantee that I will abide by all the laws, rules and

regulations of the Republic of the Philippines.
