1 Introduction

With the motive of maintaining the national Centralized Database for the Persons
with Disability, Government has developed the website to issue a Unique ID Card
for persons with Disabilities.

Administrator users at Local level, District Level, State level and National will be able
to view the pupils records as per given rights. This document demonstrates on how
to manage and maintain the website content, generate reports. Document
addresses the queries raised by various user levels for smooth operation of the
application at given point of time.

Care is taken to explain each functionality with step wise illustration and flow.

2 FAQs for Print Agency
2.1 How to export UDID card received for printing?
Print agency will be able to export the Records/application received for printing,
print and dispatch the same.
Organizations with role as “Printing Agency” will be able to print the UDID cards.

To export UDID card received for printing go to:

3.1 How does UDID Desktop app work?


 UDID Desktop Application is exclusively designed for remote locations
where the internet connectivity is not there. Desktop Administrators can
download the Exe link for the application and install the same on Desktop
system. Refer, Installation Guide.
 Administrators can download latest changes in Web DB and can Upload
the applications keyed-in by Data Entry operators.
 Data Entry Operators can enter the PwD Applications in offline mode by
collecting the details from applicants. After entering the applications, they
can view and verify the same. Verified applications can be Uploaded by
Desktop Administrator to sync with Web Database.
 Data Entry Operator can fill the form for Lost Card and UDID Card Renewal
Application.
 Desktop application provides facility to configure webcam, scanner, and
printer devices for smooth operation.

3.3 How to download data?
To upload data to server follow these steps:
STEP: 1
Login with administrator credentials:
After station code generation, the application automatically starts master data
sync process as shown below:

If not then you can manually, start downloading data by selecting


button on top right corner of the screen:

3.7 How we can make changes in uploaded data?


No, you cannot make any changes to data after upload. Not even Desktop
Application administrator can edit the data.

Only super administrator of Web application will be able to edit data after upload.

3.8 How do we know that, which data has been uploaded


successfully and which are still pending?
Remaining list of records/applications on upload application screen will be
pending for upload after Upload Data action.

3.9 How we can have status of each application when Status

You can find the status of the applicant through web application only.

2) Ensure that webcam is installed on your PC and you have configured it
through UDID settings screen

3.17 Why am I unable to edit/access menu after login?


Data must be uploaded on regular basis. If data is not uploaded data since last
seven days from specific data entry operator’s desk, then the menu will be
disabled and cannot operate after login.
Only administrator can upload the data. After that DEO (Data Entry Operator) can
operate.
For more information contact your administrator.
NOTE: ONLY ADMINISTRATOR CAN UPLOAD DATA.

3.18 Can I delete a particular record?
No provision to delete a record for Now.

3.19 How should I renew my contract?


He can contact to state or district nodal officer/ social welfare office/ CMO Office
to renew the contract.

3.20 Who is an Operator and what are his/her qualifications?


 Not specific but should be computer literate.

4 FAQs for Web Application
4.1 How to access website information?
URL: swavlambancard.gov.in

 Guest users will be able to register to apply for UDID card.


 Guest users (without registration can apply for Lost Card and Renewal of
UDID card). Registered applicants can login to access various features such
as Schemes, download forms/applications/download receipts, download
E-UDID card, and download E-Certificate.
 Logged in applicant can update personal profile.

4.2 What are procedures to get it? What I should do in order


to get UDID card?
 You need to fill the UDID card application with proper information online.
 Once the application is verified and approved, it is assigned to a specialist
for further verification.
 Visit Specialist for assessment and submit the assessment to CMO.
 After CMOs approval you will receive the Certificate ( you can download E-
Certificate)
 UDID card will be dispatched to registered correspondence address for via
post.
 You can even download E-UDID card after login.

4.3 Who create users for states? How to get access for web
portal?
National admin creates state users; user receives email with login details
(username and password).

4.4 Who create users at district level? What are access and
permission they are allowed?
State Level User will be able to create users at District level.
District level users can create users within district as Local Admin / Local Users.

4.5 Who are Dealing Officers? What is their duty?


Dealing Officers are the local users, created by either national/state/district
users.
Their responsibility may vary as per assigned role.

4.6 Who is assessment specialist?


Government Medical Practitioners, who are deputed on the task of assessing the
applicants for disability.

4.7 Who are medical board/ board specialist? What are their
duties?
Medical Board is formed by a group of Medical Officers and Specialists. Medical
Board recommends assessed applicants for Disability.

4.8 How to create user for Desktop Application?


Login with Administrator credentials
To create user for desktop application go to:

Dashboard >> Manage Desktop Users >> Add Agency/ Offices/Vendor >> Login with
Administrator credentials

A user will be automatically created for new staff (Organization, Staff, Board Members,
Specialist) and login credentials with password are sent to the user via registered email.
User compulsorily has to change the password while login for the first time.

5.8 Who is Issuing medical authority? Who is District Social


Welfare Officer?
 Issuing Medical Authority is competent medical officer for a district
authorized to issue Disability Certificate (Eg CMO/CHMO/CDMO/Civil
Surgeon). District Social Welfare Officer is district level officer to handle
the Social Welfare / Social Justice department. Disability schemes are
provided by DSO.

5.9 What if the UDID card does not get delivered to a


Resident?
 He can collect from nearest Issuing Medical Authority who issue disability
certificate.

5.10 What can the Resident do if there are spelling mistakes /


other demographic error in his/her UDID Card?
 He can return the same to Issuing Medical Authority office for necessary
corrections.

5.11 What are the UDID Guidelines for Verification that the
Verifier must keep in mind while verifying the
Documents?
 All compulsory fields should be filled correctly. Data and Names, Parent
details should be matching as per submitted Address Proof. He must verify
photo, signature, and other documents presented before him.
