Client Notification

By accepting a Manulife Agent's request for an online appointment and/or submitting the filled-up Application Form and other required forms to a Manulife Agent:

A. You allow Manulife Philippines, its employees, financial intermediaries/agents, affiliates, subsidiaries, service providers or any member of the Manulife Financial
Group to process and use all personal data* provided/secured during the online appointment, proposal, application or after-sales process for the purposes
stated in the Manulife forms and in the Company's Customer Privacy Policy found in https://www.manulife.com.ph/Customer-Privacy-Policy.

*Personal data include yours and third parties' (e.g. insured, beneficiary, contingent owner, trustee, fiancée/fiance) data; screenshot of the videoconference.

B. As Policy Owner, you confirm that:

• You are applying for Manulife insurance policy/conversion/top-up/reinstatement through Agent _______________________________________________;
• You will be paying for the premiums due on the policy and that there will be no third party payor (for transactions with premium payment of above Php500,000);
• You are in the Philippines at the time of your submission of your application form;
• You agree to be bound by the declarations/details in the Application Form, Proposal and other relevant Manulife forms;
• the information that you will provide/have provided to the Agent are correct and based on your personal knowledge and authentic documents in your possession;
and
• you will not send your personal information through social medial applications, and understand and assume the security risks in sending your information and
documents through your personal email outside of Manulife’s DocuPortal tool.
C. As Insured (If Adult Insured is different from the Policy Owner), you confirm that:

• You have given to the Policy Owner and/or the Agent your consent to provide your personal data to Manulife to process and use for this application and for the
purposes stated in the Company's Customer Privacy Policy found in https://www.manulife.com.ph/Customer-Privacy-Policy.
• You agree to be bound by the declarations/details in the policy application form and other relevant Manulife forms.
• You further confirm that the information pertaining to you as Insured are correct and based on your personal knowledge and authentic documents in your
possession;
• You confirm that you will not send your personal information through social medial applications, and understand and assume the security risks in sending your
information and documents through your personal email outside of the prescribed DocuPortal tool.
