Request to furnish information under section 6(1) of the Right to information act, 2005 on the
following questions/points.

Respected Sir/Madam,

Through this letter, I request you to provide me with a certified copy of the following information.

The following details/documents have been attached along with this letter fof your reference.

The information required that the action taken on the complaint number 1023254 (94987 94987) on
August 2nd 2022. by Mr.Karthik
Please provide the aforementioned details through the following method:

Email to the address mentioned at the end of this document.

I am willing to pay the required cost for getting the aforementioned information after deducting any
reductions under the appplicable laws. I request you to inform me in advance if the cost exceeds Rs
500/= (Rupees five hundred only).

If any specific information is to be withheld under any statutory obligation, please do inform me
about the same along with a categorized list if the information is not available in your office, kindly
forward it to the concerned public authority as per section 6(3) of the RTI Act, 2005.

I am a citizen of India and am eligible to seek information under Right RTI Act 2005.
