This is to certify that Dyesebel Panaligan, female, 55 years old from Mamburao, Occidental
Mindoro sought consult on November 6, 2022 with complaints of increased BP, vomiting, dizziness
and headache. No other associated symptoms such as fever, cough, colds, nor sore throat. Patients
vital signs were: BP- 150/90, CR-96, temp- 36. 7. No pertinent findings on physical examination.
Patient was given with Clonidine 5mg/tab once, Losartan 100mg BID as maintenance. Advised of 1
week bed rest and diet.

Diagnosis: Hypertension Stage 2 - uncontrolled
