Purpose The patient wants to request a repeat prescription for a medication
they are currently taking.
Actors Patient
Prerequisites  The patient is registered with a GP in the system
 The patient has previously been prescribed the medication
by a doctor in the telemedicine system
Narrative 1. The patient selects the medication the want to request a
repeat prescription for, from a list of their current
medication.
2. The system displays a form for the patient to confirm the
details of their request, including the quantity of the
medication they need.
3. The patient submits the request form.
4. The system sends the request to the patient’s GP for review
5. If request is approved, process the request further.
6. Sends mail to patient and generates prescription for the
medication
Outcome  The patient has received an email of approval.
 The patient has received a prescription for the medication
Alternative At step 5 if request get rejected
Alternative Narrative  Send email in which explains the reason for not approving
the request

Purpose The patient wants to register with a GP in the system

Prerequisites  The patient has not previously registered with a GP in the
system

Narrative 1. The system displays a form for the patient to enter their
personal and contact information, as well as their medical
history.
2. The patient completes and submits the form.
3. Then system verify the contact info.
4. After verification registration is completed successfully
Outcome  The system notifies the Patient that their registration has
been completed successfully.
Alternative If contact info doesn’t get verified at step 3
Alternative Narrative  System will inform patient to add valid contact info
Outcome  Once the valid contact info is entered and verified by system
registration will be completed successfully

Purpose The patient wants to request an appointment.

Prerequisites  The patient is registered with a GP in the system
 Patient has specific healthcare need or concern that requires
an appointment.

Narrative 1. The system displays a form for the patient to enter their
appointment request details, which includes the reason for
appointment and desired date and time.
2. The patient completes and submits the form.
3. Then system sends the request to doctor.
4. Doctor review the request and either approves or denies it.
5. If request is approved, system sends mail to patient.
Outcome  Patients received approval email form doctor
Alternative Request is denied at step 5
Alternative Narrative  System sends rejection email to patient
Outcome  Patients receive a rejection email from doctor with proper
reasons.

Purpose The patient wants to request a sick leave or sick note
Actors Patient
Prerequisites  The patient is registered with a GP in the system
 Patient is currently experiencing an illness or injury that
requires them to be absent from work

Narrative 1. The system displays a form for the patient to enter their sick
leave or sick note request details, including reason for the
request, the duration of the leave or note, and the contact
information for their employer.
2. The patient completes and submits the form.
3. Then doctor review the request and proceeds further.
4. If medical condition meets the criteria generate a sick leave
document
5. Send the document to patient via mail.
Outcome  Patients received sick leave document via mail
Alternative Application for sick leave didn’t meet the criteria
Alternative Narrative  System sends rejection email to patient with proper
explanation

 5. Use case Table: Referring patients to hospitals

Purpose The doctor wants to refer a patient to a hospital for further medical
treatment
Actors Doctor
Prerequisites  The patient is registered with a GP in the system
 Patient requires medical treatment that cannot be provided
by the doctor or their practice
Narrative 1. The system displays a form for the doctor to enter the
referral details, including the patient's medical condition, the
recommended treatment, and the preferred hospital.
2. The doctor submits the form.
3. The system sends the referral request to the hospital.
4. The hospital reviews the request and either approves or
denies it.
5. If the request is approved, the system sends a mail to the
doctor and schedules an appointment for the patient at the
hospital.

Outcome  Doctor receives a complete detail about his/her patient’s

appointment at hospital
Alternative Request is denied by hospital
Alternative Narrative  If the request is denied, the system sends a mail to the doctor
explaining the reason for the denial.
Outcome  Doctor receives a mail which explains the reason of
rejection.
