AFFIDAVIT OF LOSS

  I, MOHAMMED MARIANO ALA, of legal age, Filipino, single and a resident

of Mabolo Extension, Carmen, Cagayan de Oro City, after having been duly sworn
to in accordance with law, depose and state:

1. That I am a Globe postpaid subscriber in Cagayan de Oro since 2018

bearing number 0927-527-4882;

2. That I am using said Globe postpaid number for my SMS, call and GCASH
transactions since 2018;

3. That earlier today, while I was on my way to the Professional Regulation

Commission Cagayan de Oro Regional Office X (PRC-X), I lost my smart
phone (RedMi 9A) in the Jeep;

4. That the same smart phone contains all of my scanned files including my
graduation files, all contacts, school data and files;

5. That I tried to track my phone and search it in the same Jeep but to no
avail;

6. That I tried to contact my number but unfortunately it is already out of

coverage area;

7. That in spite of diligent search, the same smart phone bearing my Globe
number 0927-527-4882 could no longer be found and the same is now
beyond recovery;

8. That as such, I am executing this Affidavit of Loss to attest to the truth of

the foregoing and to support my application to change my Globe postpaid
number with Globe Cagayan de Oro so that I can retrieve or withdraw my
remaining GCASH funds from Globe Telecom Cagayan de Oro Limketkai
Center and for whatever legal purpose this may serve.
