STATEMENT of FACTS
16" November 2022
1235hrs - Agent Onboard for departure.
1245hrs - Port Clearance and Statement of Facts given.
1310hrs - Fore & Aft stations.
1312hrs - Heave anchor chain to 6 shackles on deck, leading 9 o'clock.
1315hrs ~ Single-up to 1 spring aft and 1 headline & spring forward.
1318hrs — swing out stern about 30 meters out to clear vessel astern and clearance for
swinging the vessel.
1322hrs - let go all lines.
1324hrs - all lines onboard and start heaving port side anchor. Various engine movements to
control stern about 25 - 30 meters from berth; bow thruster set to port between 30% to 80%.
1330hrs ~ with vessel perpendicular to berth at about 25 meters, anchor 1 shackle in the
water at 7 o'clock direction, rudder at hard to port, engine telegraph set to SLOW AHEAD.
133 hrs ~ engine start to move and wash created in the water to push away from berth but
not soon enough to stop the astern momentum of the vessel. The port side part of stern had
touched the corner of 1 tread of stairs on the berth.
1332hrs - continue vessel swinging to port.
1336hrs - resume heaving anchor.
1342hrs ~ anchor aweigh.
1345hrs - in position in mid-channel proceeding outbound.
1410hrs - passing breakwater outbound.
1418hrs - passing buoy no. 1.
1425hrs ~ after checking that vessel is safely out from Kashima port, had the point of contact
inspected visually to confirm seaworthiness of the vessel and informed FETRAN.
All above are TRUE to the best of my abilities and recollection of circumstances.
