I hereby agree to comply with Station’s Contests Terms & Conditions posted in the
Station Website and agree that neither me nor any of my immediate family members
will be permitted to participate in a subsequent Contest organized by Star Radio Group
within the time frame stated as below:-

( / ) 1 month (30 days) if you have won prizes worth RM200 and below;
( ) 2 months (60 days) if you have won prizes worth RM201 – RM1000;
( ) 3 months (90 days) if you have won prizes worth RM1001 – RM5000;
( ) 6 months if you have won prizes worth RM5001 – RM9999;
( ) 1 year if you have won prizes worth RM10000 and above.

In consideration of the above, I hereby undertake to indemnify Star Radio Group against
all costs, losses, damages, expenses and liabilities arising out or in connection to my participation in
the contest above mentioned.
