This is to signify our intent to avail of assistance under the Tulong Panghanapbuhay sa Ating
Displaced/Disadvantaged Workers (TUPAD) Program for fifty (50) beneficiaries
underemployed/self-employed/displaced workers in the City of Iloilo whose livelihoods were
affected by the Corona Virus Disease 2019 (COVID 19) pandemic, with funding requirement of
Two Hundred Ninety-Six Thousand Two Hundred Fifty Pesos Only (Php 296,250.00).

We have attached the copy of the TUPAD Work Program (Enhanced OSEC-FMS Form No. 3)
and other documentary requirements relative to our request.
