Definition - unique steroid hormone that can modulate


calcium haemostasis directly or thru its effects on the
differentation & development of the various calcium
regulating cell systems

Vit D3 - must be hydroxylated in liver then in kidney

Kidney - inc PCT reabsorption of phosphate


GIT - inc active transport of Ca
Bone - not fully understood
- stimulates osteoblasts to secrete osteoclast-activating
substance Osteoprogetegerin ligand (OPGL)
-> activates osteoclasts & induces bone resorption

Vit D dependent Rickets


Type 1 - lack of renal enz to hydroxylate D3
- tf only have 25-hydroxyD3
- treat c large doses of calcitriol daily -> get nephrocalcinosis

Type 2 - receptor resistance


- have high levels of calcitriol

Osteoporosis
- pts c osteoporosis have dec absorption & dec calcitriol levels
- thought that ~ 10% of osteoporosis pts have low vit D3 levels
- conflicting evidence on role of calcitriol in osteoporosis
