Certified Company: Certification Application Template

Instructions:

For us to verify your employees’ active certifications, we will need each candidate’s full name, email, CFID, and Active Certification. The CFID is the unique
personal identifier that helps us validate an individual’s certification and is the Candidate’s ID on CertMetrics. Please use this template and follow these
instructions for providing CFIDs:

- Copy this Certified Company CFID Application template into a separate Google sheet
- Edit the newly created Google sheet and include each candidate’s full name, email, CFID, and Active Certification
- To find CFID, each candidate must navigate to CertMetrics using the link here. The CFID can be found on the top right of the screen.
- Once all candidates and their CFIDs have been added, please ensure the Google sheet is open for Viewing and Editing before submitting. This will
ensure we can properly verify candidates that were provided.
- Now that you have your Google sheet with each candidate’s full name, email, CFID, and Active Certification added, provide the link to your CFID
Application sheet and submit your application at the Certified Company Application Page
